Abstract
Objective. To assess the prevalence of tachycardia-induced cardiomyopathy (TCMP) and identify predictors for the development of TCMP in children with idiopathic ventricular arrhythmias (VA).
Material and methods. Data of 392 children with idiopathic VA, who were examined and treated at the Almazov National Medical Research Center in 2011–2023, were retrospectively assessed. Group 1 included 35 (8.9%) patients with TCMP, group 2 – 357 (91,1%) patients without TCMP. All patients with TCMP underwent echocardiography evaluation 2 and 6 months after the start of antiarrhythmic therapy or catheter ablation.
Results. The prevalence of TCMP in pediatric patients with idiopathic VA was 8.9%. The study identified significant risk factors for the development of TCMP in children with idiopatic VA: age ≥ 12 years (p = 0.008), fatigue (p = 0.040), presyncope/syncope (p = 0.035), coupled premature ventricular contractions (PVCs) (p = 0.010) and ventricular tachycardia (VT) (p = 0.002). ROC analysis revealed that ventricular ectopy density ≥ 36.8% is associated with the development of TCMP (p < 0.001) with sensitivity 76.5% and specificity 78.1%.
Conclusion. Our study demonstrates, that age ≥ 12 years, presence of symptoms, coupled PVCs, VT, and VA density ≥ 36.8% are associated with the development of TCMP in children with idiopathic VA. These factors can be taken into account when deciding whether to treat children with idiopathic VA.
