Abstract
Due to the success of cardiac surgery, most patients with congenital heart disease (CHD) live to adulthood. Although the survival rate of patients with isolated simple malformations is close to that of the general population, patients with moderate to severe CHD are at risk of cardiac, concomitant pathology and premature mortality, and therefore many patients require lifelong follow-up, regular examination and treatment by qualified specialists. The key to a successful prognosis of the disease in this category of patients is the continuity and continuity of medical care.
Currently, most of the growing children with CHD are insufficiently informed about their disease, about possible complications and prospects associated with CHD, about the need for medication and dynamic monitoring. There are also significant gaps in the field of healthy lifestyle education, knowledge about the necessary physical activities, in matters of choosing a profession and starting a family. As you know, parents of children with CHD are not always ready for the transition period. To facilitate this stage, transition programs aimed at adolescents and their families with the participation of a multidisciplinary team of specialists in the field of CHD are needed. Using international experience, as well as taking into account the peculiarities of the healthcare system in the Russian Federation, it is promising to develop our own programs that could be implemented within the framework of cardiac rehabilitation programs based on large Centers with experience in the treatment of CHD in all age groups.
