Аннотация
Проанализированы причины развития отдаленных осложнений у пациентов с транспозицией магистральных артерий (ТМА),
которым выполнена операция артериального переключения. Выявлено, что основными факторами риска, по данным разных
авторов, являются особенности анатомии магистральных и коронарных артерий, их пространственное взаимоотношение
и хирургические особенности выполнения операции, особенно реимплантации коронарных артерий. Факторы риска
летального исхода в послеоперационном периоде, по данным многих исследований, включают в себя объем опыта лечебного
учреждения, уровень оперирующего хирурга, вес пациента, взаимоотношение аорты и легочной артерии, степень гипоплазии
левого желудочка, наличие обструкции выводного отдела левого желудочка и аномалий коронарных артерий:
интрамуральное расположение и единственная коронарная артерия. Учитывая недостаточную информацию о влиянии
разных факторов риска на развитие осложнений в отдаленном периоде, а также отсутствие алгоритма динамического
контроля патологических состояний, свойственных этой категории пациентов, не вызывает сомнений актуальность
выполнения исследований в этом направлении для анализа результатов лечения детей с простой ТМА, изучения патогенеза
и определения факторов риска развития осложнений у данной группы пациентов, особенно в отдаленном периоде.
