Abstract
Tetralogy of Fallot with pulmonary atresia is a complex congenital heart defect. Tactics of treatment depends on the severity of
anatomic disorders and morphology of pulmonary blood flow. Two-thirds of patients have a large aorto-pulmonary collateral arteries
(APCA), patent ductus arteriosus as a source of pulmonary circulation. Rare blood supply to the lungs through the aorta-pulmonary
window, saved 5th aortic arch or fistula of the coronary arteries is rarely observed. Clinical observation demonstrates a rare source of
pulmonary blood flow – collateral vessels extending from the aortic sinuses – which helps to showcase the capacity of special methods
of diagnostics, to start the discussion on aspects of occurrence and treatment according to the literature.
