Abstract
Introduction. Isolated congenital aortic stenosis is represented by a wide range of both anatomical and clinical options. The most common is stenosis of the bicuspid aortic valve, with a significant portion of patients requiring early surgical intervention. Objective. To evaluate the results of reconstructive operations on the aortic valve in comparison with the operation of Ross (Ross–Konno) in children.
Material and methods. The work is a retrospective comparative study, including 130 patients who underwent surgical treatment for congenital aortic valve stenosis from November 2008 to November 2019 at the Federal Center of Cardiovascular Surgery of the Russian Ministry of Health (Penza). All patients were divided into two groups: group 1 included 94 patients who underwent various reconstructive interventions on the aortic valve, group 2 included 36 patients after the Ross and Ross-Konno operations.
Results. Early hospital mortality in group 1 was 1.1% (n = 94), in group 2 – 5% (n = 36) (p = 0.35). In group 1, reoperations were performed in 8 (8.4%) cases, of which in 2 (2.1%) cases Ross–Konno surgery, in 2 (2.1%) cases, repeated aortic valve repair, in 2 (2.1%) prosthetics of the aortic valve and in 2 (2.1%) cases excision of the subaortic membrane. In group 2, repeated interventions were required in 3 (8.1%) cases, of which mitral valve prosthetics were performed in 1 (2.7%) patient, tricuspid valve repair in 1 (2.7%) patient and subaortic obstruction in 1 (2,7%) case.
Conclusion. Reconstructive valve surgery will be increasingly viewed as a standard for patients with aortic valve disease, gradually replacing other, various treatment methods as the first stage.
