Abstract
Objective. Is to analyze the frequency of development of complications in the long-term period in patients after arterial switching
operation, as well as to identify risk factors for the development of these complications.
Material and methods. The analysis of the results of treatment and dynamic observation of patients undergoing arterial switching
(ASO) surgery in connection with the transposition of the main arteries (TGA) was performed. The study included the completed
cases of surgical treatment of 248 patients with TGA who were operated at the clinic of Bakulev National Medical Research Center of
Cardiovascular Surgery.
Results. ASO is accompanied by the risk of developing a number of complications. In our study, coronarogenic complications in the
long term were detected in 14.1% of cases. Neoortic valve insufficiency (ICH) in the postoperative period developed in 104 patients
(41.9%). Dilatation of the neoortic root is quite common among operated patients for TMA and is often combined with ICH. The risk
of developing obstruction of the output section of the right ventricle, stenosis of the pulmonary artery and/or its branches in operated
patients, according to the data of various authors, is 17–26%. In our study, the incidence of this complication was 19.4%
(48 patients). As a result of the analysis of the frequency and structure of the development of neurological complications, it was
revealed that the intellect coefficients of the studied cohort were close to normal.
Conclusion. When analyzing the results of echocardiography, clinical examination data and other instrumental methods for verifying
complications, statistically significant predictors of the development of each of these complications were revealed. These risk factors
for the development of the above-described complications determine the need for correction in treatment tactics and dynamic observation
in order to minimize the risk of their occurrence in the remote period after the arterial switching operation. In addition, it is
obvious that all the pathophysiological mechanisms of the development of these complications are interrelated and the impact on
each risk factor reduces the risk of several complications.
