Abstract
Objective. Evaluation of the life quality among the children having different congenital heart diseases (CHD) and being operated
radically in the first year of life in conditions of artificial blood circulation.
Material and methods. This work is a retrospective comparative study, which includes 669 patients being operated within November
2008 to February 2018 in conditions of artificial blood circulation. The criteria considered in the research are either indications for
radical correction surgery of CHD under conditions of artificial blood circulation or the age of the patients at the moment of operation
which is up to 1 year, including the neonatal period. All patients were grouped into two types: Type I included 217 (32.4%) patients
with CHD accompanied with cyanosis, while Type II included 452 (67.6%) patients with CHD of the so-called “pale” type. All Types
of patients underwent radical correction surgery. Age of children at surgery moment ranged from 0 to 1 year (median – 3.5 months,
25 percentile – 5 day, 75 percentile – 8 months). The observation period after surgery took from 4 to 8 years (on the average
4.68±2.6 years). Pediatric Quality of Life Inventory (PedsQL) 4.0 Generic Core Scale and PedsQL Cardiac Module 3.0 Questionnaire
was used to define life quality.
Results. 669 patients with different CHD after radical correction surgery in conditions of artificial blood circulation have been studied
for life quality. 537 (80.3%) patients have high level of life quality. In Type I the index amounts to 155 (71.4%) patients, while in
Type II the index amounts to 382 (84.5%) patients. 78 (11.6%) patients have moderate index decrease. Among them there are
39 (18%) patients in Type I and 39 (8.6%) patients in Type II; 54 (8.1%) patients have been marked with the index decrease in Type I,
i.e. 23 (10.6%) patients, while in Type II there are 31 (6.9%) patients. It has been stated that both Types of heart diseases do not have
statistically significant differences at level p=0.4–0.6.
Conclusion. Surgical treatment results of CHD, considering patient's life quality, allow to obtain some important data and uncover
main factors responsible for index decrease. According to our survey, radical anatomical correction surgery provides acceptable life
quality for most patients. Evaluating and comparing life quality of children with CHD, being operated radically in the first year of life
in the analysed Type groups, we have found no statistically significant differences.
