Abstract
The purpose is the analysis of cardiovascular manifestations paediatric inflammatory multisystem (PIMSS) syndrome associated with COVID-19 in the Republic of Mordovia.
Material and methods. The article provides a brief literature review and analysis of our own observations on the nature of changes in the cardiovascular system in patients with an established diagnosis of PIMSS in the Republic of Mordovia who received treatment at the Mordovian Children's Republican Clinical Hospital in the period from April 2020 to February 2022, and also a description of a clinical case of myocarditis during paediatric inflammatory multisystem syndrome associated with COVID-19 in the teenager.
Results. The defeat of the cardiovascular system was diagnosed in all 30 patients with paediatric inflammatory multisystem syndrome associated with covid-19. The 25 (83.3%) patients had myocardial dysfunction and/or dilatation of the heart chambers; decrease in left ventricular contractility was observed in 18 (60%) patients, mitral and/or tricuspid regurgitation occurred in 11 (36.7%), pericardial effusion in 5 (16.7%), shock requiring inotropic support was observed in 11 (36.7%) children, dilatation of the coronary arteries (<2.0_) in 3 (10.0%). Most children 18 (60.0%) had sinus tachycardia, 3 (10.0%) children had ectopic arrhythmias and conduction disturbances, and 21 (70.0%) had changes in repolarization. All children showed an increase in markers of inflammation, hypercoagulability, and an increase in the level of cardiac enzymes.
Conclusion. Damage to the cardiovascular system in patients with paediatric inflammatory multisystem syndrome is characterized by polymorphism and significant for determining treatment tactics and prognosis.
