Abstract
Objective. The aim of the study is a comparative assessment of normothermic selective antegrade cerebral perfusion and hypothermic
techniques in reconstructive operations on the aortic arch in newborns and infants.
Material and methods. From October 2008 to April 2018, 73 consecutive patients were underwent reconstructive operations on the
aortic arch in Federal Cent32 patients, who were operated under the conditions of normothermia and 41 patients (group 2) with hypothermia (mean rectal
temperature in group 2 was 27.4±3.1 °C with range 20–32 °C).
Results. Еarly postoperative mortality were founded in both groups and rate was 2 (6.2%) patients from group 1 and 3 (7.3%)
patients from group 2 (p=0.23). During the study, statistically significant were obtained in a such number of parameters as: the duration
of cardiopulmonary bypass was significantly higher in operations with hypothermia (102.7±14.2 and 237.8±33.0; p=0.028),
respectively, the reperfusion time (25.1±9.6 and 54.3±24.1; p=0.003) and the operation time (202.4±29.2 and 283.4±59.6;
p=0.007), also, the level of lactate after surgery was significantly lower in the group of patients who underwent operations under normothermia
conditions (3.6±1.3 and 7.5±3.1; p=0.016).
Conclusion. The normothermic selective antegrade cerebral perfusion has results similar to hypothermic perfusion, but the possibility
of reducing bypass time can be potentially advantage. However, normothermic selective antegrade cerebral perfusion in complex
surgery has a multifactorial effect and it must be evaluated in more patients with longer follow-up than was performed in our study.er for Cardiovascular Surgery (Penza). Patients were divided into two groups. The first group included
